Anyone who has experienced kidney stones will know that it’s one of the most excruciating conditions you can have, and unfortunately it’s a common problem here in Indonesia. The effects can vary from shooting back pain to intense pain while urinating.

A kidney stone is a solid piece of material that forms in a kidney when there are high levels of certain substances in the urine. Among others symptoms of kidney stones are cloudy or foul-smelling urine, nausea and vomiting, fever, and a persistent need to urinate. To treat this condition, special medical treatment should be undertaken immediately before the symptoms worsen.
Luckily with the help of technology, kidney stones can now be treated quickly and painlessly with a special method called Retrograde Intrarenal Surgery (RIRS), which is known as the newest technique for kidney stone removal. RIRS renders smaller kidney stones more accessible and upper urinary tract tumors treatable. RIRS was first used to treat small kidney stones, the approach later attracted a great deal of attention and it was suggested that larger stones could also be treated. “RIRS uses an endoscopic instrument called a flexible uteroscopy. The tool is inserted through the urinary tracts; the urethra, bladder or ureter, until it goes through to all parts of kidney. The combination of flexible ureteroscopy and laser maximize the effectiveness of the RIRS method to cure this condition,” explained Dr. Arnold Muara Simanjutak Sp.U, the Urology Specialist at Premier Jatinegara Hospital. Since it is considered as relatively new and improved method, RIRS is not necessarily used as the first treatment for all kidney stone problems. “Before deciding the treatment, the patient needs to consult the doctor as there are several tests that must be undertaken to diagnose kidney stones. Some simpler cases can be handled by ESWL (Extracorporeal Shock Wave Lithotripsy). However, if the kidney stone’s diameter is larger than two centimetres, or it is located in a difficult position, RIRS will be suggested,” added Dr. Arnold.
Any endoscopy approach has side effects, including RIRS. According to Dr. Arnold, several side effects need to be anticipated; mainly urosepsis and infection, however these conditions can be prevented with antibiotics before the RIRS procedure. RIRS has been designed to complement the shortcomings of previous methods. Before treatment, three factors should be considered; the indication, the availability of tools and of course the qualified medical personnel performing the procedure. “The success percentage of RIRs is quite large, approximately 95%,” concluded Dr. Arnold.
